Community Health Needs Assessment (CHNA) 2022
The Community Health Needs Assessment is a data-driven report that examines the health status, behaviors, and needs of residents within Hartford HealthCare’s service areas. It helps identify key public health concerns, health disparities, and gaps in access to preventive services. The findings guide strategic decisions and support initiatives to improve overall health outcomes, extend life expectancy, and enhance quality of life in our communities.
Community Health Improvement Plan (CHIP) 2022–2025
The CHIP outlines Hartford HealthCare’s multi-year action plan to address the priority health issues identified in the CHNA. Developed in partnership with local stakeholders, this plan focuses on targeted interventions, health equity, and expanding access to care.
